What does 5D actually add over 4D?

Each extra dimension gives the rollers another axis of movement. A 4D system varies its depth and rhythm as it travels; a 5D system adds further freedom on top of that, so the motion reads as more hand like and less mechanical. Running both together is what makes this the flagship.

Do I have to learn the programmes?

No. Sit down, start the AI session and the chair scans your body and builds the session itself. The manual controls are there when you want them, but the chair is designed so the first use needs no study.
